About Us: A Healthcare Home is not a place of residence and it is not a home health program. It is a place where individuals receive higher quality of care and achieve healthier outcomes. Healthcare home is wellness program comprised of a team of healthcare professionals committed to improving the health and wellness of clients facing chronic diseases. Every client will work with a Nurse Care Manager on managing health conditions for the goal of increasing knowledge and improving health outcomes.
· The Role of a Nurse Care Manager: The Role of a Nurse Care Manager: A Nurse Care Manager (NCM) is available to help you identify and achieve your healthcare and wellness goals. A NCM promotes and encourages wellness coaching, healthy lifestyle choices, and preventative care. They also teach clients how to better manage their chronic health conditions, assist with connections to primary care providers including Swope Health, provide tests for cholesterol, blood glucose, and Hemoglobin A1c (an indication of diabetes control), and they aid in blood pressure and weight management.

Q: Does this cost me anything? I don’t have insurance.
A: No, HCH services are no cost to you. While this program is available to Missouri HealthNet participants who meet eligibility requirements for admission, Beacon’s unique Healthcare Home Non Medicaid program provides identical wellness promotion to those without current benefits. Both are available without cost to participants. The expectation for both programs is improved health outcomes with a reduction in healthcare costs. It is important to note that primary care services are not provided by the HCH, though it is the responsibility of the HCH to promote desirable and effective outcomes through the coordination of care with outside providers.
Q: How often do I have appointments?
A: Appointments with your NCM are once a year or are as needed according to your preference. The appointments generally occur on a quarterly basis though we value your opinion as to whether you would like to be seen monthly, bi-annually or annually.
Q: Do you offer in home visits to clients?
A: Beacon’s Healthcare Home is not the same as in-home nursing care or a home health. Though the Nurse Care Managers visit some consumers in their homes for assessments and education, most appointments are scheduled at one of Beacon’s locations in Clay, Platte, and Ray counties.
Q: What can I expect from an appointment with my NCM?
A: An annual health screening will be done along with vital signs and metabolic syndrome screening labs if you haven’t had this done within the past year.
