Hospital Diversion

Hospital Diversion is a collaboration between Hospitals and Community Mental Health Centers

The Hospital Diversion Program is a collaboration between hospitals and community mental health centers who work together to divert individuals with psychiatric and/or substance use disorders from emergency rooms and hospitals to alternative services in order to reduce the number of hospitalizations.

The program provides intensive outreach case management to assist individuals stabilize and identify resources in their community in hopes of diverting future crises.
