News Release – Tri-County Mental Health Services was awarded a $52,000 grant for the Health Care Home program from the Health Forward Foundation, formerly the Health Care Foundation of Greater Kansas City. The foundation received 46 grant applications and Tri-County Mental Health was the fourth largest of the 13 selected in November of 2018.

Tri-County’s program selected for the grant is the Health Care Home program. It is a person-centered system of care that facilitates access to primary and acute health services, behavioral health care and long-term community-based services. The program is founded on the belief that helping people lead healthier lives can greatly reduce the negative impacts of living with long-term mental illness, substance use and medical conditions.

People who come to Tri-County get a broad range of health care screenings and education. From tobacco cessation to metabolic screenings, and even help with healthy eating, the Tri-County staff members working in the Health Care Home program help consumers avoid and address the multiple health issues that often co-occur with mental illness. They provide the services at Tri-County, where the consumers have easy access in a comfortable, familiar setting. The grant will extend the benefits of the Health Care Home program to those without insurance or Medicaid.

Health Forward Foundation is igniting a culture of health by tackling pressing health issues for those most in need. Through partnership and advocacy, they are working to transform communities so everyone has an opportunity for better health. Since it began giving grants in 2005, Health Forward has awarded approximately $260 million to agencies addressing health needs in the community.
